The survey does not collect your name, your pet's name, or any clinic or veterinarian names.
Survey responses are collected and stored through Google Forms. Contact form messages are received through our email provider.
Because the survey is anonymous, submitted responses cannot be individually identified, withdrawn, or deleted after submission.
We do not sell or share your information with third parties.
Aggregated, de-identified survey results, and de-identified paraphrased excerpts from open-text answers, may be used in HAIS-Vet reports, toolkit updates, or educational materials.
