• The survey does not collect your name, your pet's name, or any clinic or veterinarian names.

• Survey responses are collected and stored through Google Forms. Contact form messages are received through our email provider.

• Because the survey is anonymous, submitted responses cannot be individually identified, withdrawn, or deleted after submission.

• We do not sell or share your information with third parties.

• Aggregated, de-identified survey results, and de-identified paraphrased excerpts from open-text answers, may be used in HAIS-Vet reports, toolkit updates, or educational materials.
