Yes, we would like to hear from you. HAIS-Vet is developing a Phase 2 program offering clinics free, practical quality-improvement resources.
Your responses will be combined with those of other participants and reported only in aggregate format. Individual responses will not be identified or attributed to you. If you provide an answer to an open-ended question, a brief excerpt may occasionally be included in a report or toolkit update in a de-identified, paraphrased form, with all, if any, identifying information removed.
